"Lighting Up New Horizons for Disadvantaged Students" Eyeglasses Subsidy Program

Forwarding the "Lighting Up New Horizons for Disadvantaged Students" Eyeglasses Subsidy Program by the Taipei City Yu-Yang Charity Foundation.

I. In accordance with the letter from the Taipei City Yu-Yang Charity Foundation, Yu-Yang Letter No. 1150020018, dated February 24, 115.

II. The Taipei City Yu-Yang Charity Foundation, understanding that students from impoverished families lack the financial means to afford eyeglasses, leading to academic setbacks or difficulty concentrating in class due to poor vision, is organizing the aforementioned subsidy program. If our school has students whose families are unable to afford eyeglasses, they may prepare the application materials and submit them to the foundation.

III. The deadline for submitting applications for the second semester of the 114 academic year for the aforementioned subsidy program is April 30, 115 (postmark date applies). Application conditions and notes:

(I) The subsidy primarily covers the cost of "eyeglasses" and does not include contact lenses.

(II) Applications should be submitted in a consolidated list by the school. The foundation does not accept individual applications.

(III) Priority will be given to applications for eyeglasses purchased at partner Universal Optical stores referred by the foundation. If it is not possible to purchase eyeglasses at a partner store, please call the foundation to inform the review committee of the reason for further review. Contact number: 02-2711-4888.

(IV) Eligible applicants are under 18 years of age, primarily students in high school or below.

(V) Schools wishing to apply should prepare the required documents as described in Section V, items (I) to (IX), and mail the hard copy documents to the foundation's "Eye Protection Program Review Committee." Upon approval of eligibility, a dedicated person will contact the individual case's parents to assist with appointment scheduling and the reimbursement process.

IV. The required application documents are as follows:

(I) Required documents: A submission list from the school or social welfare organization.

(II) Individual application form for each case. The school must fill in the "Unit Opinion or Review Explanation" column.

(III) A copy of the new household registration or a copy of the household registration transcript valid within one year (including all notes).

(IV) Proof of low-income, low-middle-income household status, or proof of financial hardship for the applicant's family.

(V) A copy of the cover of the savings passbook for the remittance account where the subsidy will be disbursed (the remittance account should be in the applicant's name).

(VI) School vision screening report or diagnostic information from an ophthalmologist or eye clinic indicating poor vision for the individual case.

(VII) Student appointment form for the eye protection program in cooperation with Universal Optical Group (for partner optical stores). Please fill out the appointment form and submit it with other documents to the foundation. If approved, a dedicated person will contact the individual to schedule an appointment.

(VIII) Personal data collection, processing, and usage consent form (agreeing to have case stories anonymized and shared on the official website for public acknowledgment).

(IX) Applicants must personally sign the foundation's receipt form when submitting the application.

(X) After the eyeglasses are fitted and the invoice is obtained, please mail the original invoice for the eyeglasses cost to the foundation. Disbursement will be made after review and confirmation. Please also provide photos of the student's life and learning situation before and after getting eyeglasses for future review by the competent authority.

V. For any related questions, please contact the Taipei City Yu-Yang Charity Foundation (Phone: 02-2711-4888). Schools should consolidate the required application documents and mail them to: "Eye Protection Review Committee, 12F-1, No. 60, Section 4, Zhongxiao East Road, Daan District, Taipei City 106070."

VI. After the foundation's review committee confirms the application documents and eligibility, they will conduct interviews and assessments. Approved applications will receive a discretionary subsidy amount (not the full invoice amount). Submitted documents, whether approved or not, will not be returned.
