※※Student Group Insurance Claim Procedures and Notes※※
I. Claims can be filed after the child's recovery, but no later than 2 years from the date of the accident.
II. There are three ways to submit claims:

 (1)*School Submission (The school health center will notify Cathay Life Insurance personnel to collect the documents; claim processing time is longer).

 (2)*Agent Delivery (Faster claim processing): Please contact Ms. Chang Ya-Li, Cathay Life Insurance District Manager (Please inform her that it is a claim for a Shijian Elementary School student). Ms. Chang will personally assist you. Ms. Chang's phone number: 0988-298220.

 (3)*In-person submission at a Cathay Life Insurance service location (Faster claim processing).

 III. Claims are only processed if the actual expenses exceed NT$500 after deducting registration fees and diagnostic report fees.
 IV. Required Documents:
(1)Student Group Insurance Claim Form (The form can be obtained from the health center or downloaded from the website)
＊All fields marked with a red asterisk (＊) must be filled out (policy number is not required).
＊Fill in the insured's name (student), ID number, gender, date of accident, and cause of accident.
＊Specify the remittance method (remittance to the beneficiary's account or legal guardian's account): Fill in the beneficiary's name, relationship, and ID number.

 (2)Medical receipts (Photocopies or duplicates must be stamped with the hospital's official seal and "Certified True Copy").

 (3)Diagnostic certificate (Photocopies must be stamped with the hospital's official seal and "Certified True Copy").
 (4)Photocopy of the beneficiary's or legal guardian's bank passbook cover.

 (5)Photocopy of household registration (including detailed records; records cannot be omitted).

 (6)If you have low-income household status, middle-income household status, or a serious illness card, please attach a photocopy of the relevant certificate.
V. If the student visited two hospitals for the same accident, please attach diagnostic certificates from both hospitals.
VI. Student group insurance adopts an actual expense reimbursement method. For each incident, [diagnostic reports and registration fees are not covered], and outpatient visits for illness are not covered. The maximum coverage for accidental outpatient visits is NT$5,000; the maximum coverage for hospitalization is NT$50,000.
※ The payment method for hospitalization medical insurance or injury outpatient insurance is based on the actual medical expenses paid according to the receipt, after deducting excluded items (such as registration fees, diagnostic reports, etc.). If the amount exceeds the deductible of NT$500, insurance benefits will be paid within the insured amount. Amounts below NT$500 will not be paid (except for low-income, middle-income, and serious illness card holders).
